Please submit a 150-word or more, typed entry. You must include your name, mailing address, and phone number. One entry per person. Contest begins Nov. 1, 2018 and ends Feb. 14, 2019. Entries must be submitted online or emailed with confirmed receipt by Feb. 14, 2019. At least one independent party will judge the entries. Previous winners and Shield HealthCare (Shield) employees are ineligible. Shield may publish and use submitted content for publication at no cost. Shield will notify contest winners by Feb. 25, 2019.
